In The Sims 4, you cannot create entirely custom neighborhoods from scratch. The game provides predefined neighborhoods or worlds where you can build and customize individual lots within those neighborhoods. The available neighborhoods in the base game and expansion packs come with their own unique characteristics, environments, and pre-built lots.

However, you do have the freedom to modify and customize the existing neighborhoods and lots to suit your preferences. You can bulldoze and rebuild lots, edit existing buildings, change lot sizes, and decorate the surrounding areas within the limits of the game's world-editing tools.

Additionally, the "Get to Work" expansion pack introduced the ability to create and customize retail lots, where you can design and manage your own shops and businesses.
